Data Security Incident Notice

SEATTLE, October 19, 2022 /PRNewswire/ — Country Doctor Community Health Clinic (“CDCHC”), a not-for-profit healthcare provider, has been made aware of a data security incident that may have impacted data belonging to certain employees and patients.

On January 6, 2022, CDCHC has discovered unusual activity in its digital environment. Upon learning of this, he immediately took steps to secure his digital environment and investigate. The CDCHC also hired independent cybersecurity experts to conduct an investigation. As a result of this investigation, CDCHC learned that an unauthorized actor had accessed certain files and stored data in its systems.

Although CDCHC has no evidence that any information potentially involved in this incident has been misused, out of an abundance of caution, CDCHC is advising those affected of steps they can take to help protect their information. CDCHC also provides credit and identity protection services to people whose Social Security numbers have been affected. The information potentially affected varies by individual, but may include: name, address, social security number, date of birth, and protected health information. On October 14, 2022CDCHC has completed the process of identifying the affected individuals’ current address information in order to effect written notification of the incident by US mail.

CDCHC took action in response to this incident and made changes to its cyber environment to help prevent similar incidents from happening in the future.

CDCHC has set up a toll-free call center to answer questions about the incident and to address related concerns. Call center representatives are available Monday through Friday between 6:00 a.m. – 6:00 p.m. PST and can be reached at 1-833-875-0659.
